Our fees are based on the current Ontario Dental Association Fee Guide

Dental Insurance

Patients with dental insurance are asked to be sure to provide the office with all the details of their plan including, at a minimum:

  • The name and date of birth of policy-holider
  • Name of employer
  • Name of insurance company
  • Plan number
  • Division
  • Dependent code
  • Patient relationship to policy-holder
  • Patient’s date of birth

Additional information that is important for the patient to know and is best shared with the office includes:

  • Fee guide year used by your plan
  • Details of plan coverage (types of treatment covered, percentages)
  • Annual maximums

In most cases today, we are able to submit claims electronically.  When possible, our office awaits payment directly from the insurance company and then bills our patients for the amount not covered.

Even if the office is able to accept payment directly from your insurance company, each patient is financially responsible for the fees associated with the treatment he or she receives. As coverage varies from one policy to another, we cannot guarantee that 100% of all fees will be covered.

Insurance claims are submitted immediately after your appointment.  If we have not received payment by the 61st day after treatment, the patient is then billed in full. Your account must be kept current or the office will no longer be able to accept payment directly from your insurance company.

Non-insured services

Payment of non-insurance dental fees is due upon completion of service. Our office accepts payment by cash, Visa, MasterCard, AMEX, Interac, and e-transfer. For major treatment, special financial arrangements can be made with one of our account managers. Your dentist will provide you with an outline of the recommended treatment and the fees involved with this treatment.

 

Overdue accounts

Please be advised that the policy of this office is: regardless of insurance involvement, all accounts with balances over 60 days will have interest applied at a rate of 2% per month (24% annually). For returned cheques, a handling fee of $40.00 will be levied.
